A Fresh Look at Frozen Eggs: Progress on Egg Banking
While significant enhancements in technology have brought about much improved success rates following freezing (cryopreservation) of embryos, results following egg freezing have been very disappointing. This is the reason why, since the first pregnancy from frozen eggs was reported by Chen in 1986, only about 1000 frozen-egg births have been documented worldwide. Yet, hardly a month goes by where we do not hear or read about the great benefits associated with egg banking and how by freezing their eggs, women could confidently exercise control over their reproductive performance. But presently such claims belie reality. It is not uncommon to see advertisements of a 30-40% birth rate per transfer procedure using embryos derived from previously frozen eggs... But this statistic is “empty” because it does not tell the whole story. It fails to address the fact that in the final analysis each egg being frozen and banked will have no more than a 4% chance that it will ... Read more

The Perkins family in Texas delivered sextuplets last month. We congratulate the new parents and hope their newborns will all do well. They are going to be busy. Like many couples struggling with infertility, they sought help from a fertility clinic. It is pretty certain they did not set out to have sextuplets when they … Read more
In vitro fertilization (IVF) treatment is expensive and virtually no one contemplates undergoing treatment without first considering this. But with IVF there are really two components to this cost. The first is financial and the second (usually the most burdensome) is emotional. The Financial Cost of IVF “Perception is reality.” Nothing could be truer when … Read more
The process of going through an IVF stimulation cycle is hard to endure at the best of times. The strange mixture of medications, ultrasounds and the assorted probings and proddings which make up the cycle can be overwhelming at times. The process itself initially seems like a mixture of rocket science and magic and can … Read more
When I walk out to the waiting room to call upon my patient for their appointment, I can always pick out the “first timers”. They are generally a couple, huddled in two corner seats, wide-eyed, with an anxious body posture. This always bothers me, because there is no need to be anxious or worry about … Read more
“Unexplained IVF Failure” Please join Dr. Geoffrey Sher for a live webinar where he’ll discuss the three Root Causes of IVF Failure: Stimulation Protocols – how an individualized protocol of controlled ovarian stimulation can influence egg quality/quantity and contribute to IVF success. Egg/Embryo “Competence” – how chromosomal abnormalities of the egg/embryo can lead to repeated … Read more
(CLICK CHART TO ENLARGE) In a field that is virtually devoid of regulation in the U.S., there is a real need for access to accurate and reliable IVF outcome statistics. But alas, this is not readily available. In fact, for over 20 years, the American Society of Reproductive Medicine (ASRM) and the Society for Assisted Reproductive Technology (SART) … Read more
“Little of how we conduct IVF treatment is based on findings derived from “gold standard” randomized and controlled studies. They simply do not exist and, given the multitude of variables that influence IVF outcome, such studies would be virtually impossible to conduct reliably anyway. In fact if a book on IVF were to be constructed … Read more
Here are a list of points I recommend that patients consider when contemplating tubal ligation reversal. One should be wary of what the proclaimed “success rates” are for tubal reversal; “success” should be measured in terms of live birth not just patency of the tube after surgery. The tubes may be open but not functionally normal; there … Read more
(This is the first in a two part series detailing my approach to IVF at SIRM.) Little of how we conduct IVF treatment is based on findings derived from “gold standard” randomized and controlled studies. They simply do not exist and, given the multitude of variables that influence IVF outcome, such studies would be virtually impossible to … Read more
Many doctors perform a laparoscopy for infertile women or “want to check that everything is ok prior to IVF”. Unfortunately, while it sounds effective, this is another useless procedure. I advise you to avoid it, no matter what your insurance covers. The reasons are simple: most of the time nothing is found, and even when … Read more
The issue of whether to apply CGH technology to the testing of day-3 versus day-5 embryos and/or whether metaphase CGH (mCGH) or array CGH (aCGH) should be the preferred method used for testing eggs and embryos for their “competency” continues to rage. Central to this debate are several considerations. Fresh versus Frozen embryo transfers of CGH tested embryos:Is it … Read more
by Walid Saleh, MD The short answer: It depends when they are measured. 1) Early Pregnancy Because serum progesterone is a reflection of progesterone production by the corpus luteum, which is stimulated by a viable pregnancy, it is helpful as a screening tool to rule out an abnormal pregnancy like a miscarriage or tubal pregnancy. … Read more
